A case of sexual addiction

Complex cases characterised by sexual addiction and paraphilic behaviours can be analysed and treated
on the basis of conceptualisation that takes into account the patient’s interpersonal motivational systems
and metacognitive functioning. In order to support our views, we have referred to the interpersonal
motivational systems model developed by Liotti and collaborators and to metacognitive functioning as
conceived by the «Terzo Centro di Terapia Cognitiva» (Third Centre of Cognitive Therapy).
Besides a general description of the patient, two transcripts are presented in detail to show how the
interpersonal motivational systems and metacognitive functioning interact to sustain the patient’s
sexual perversions and, moreover, to show how sexual dynamics are, in reality, part of a broader
relational style. In addition, the therapeutic interventions are highlighted in order to show how
conceptualisation can guide work in psychotherapy.
