Disturbo da uso di cocaina: trattamento cognitivo-comportamentale

This paper describes cognitive-behavioral treatment in a 43-year-old man with cocaine use disorder, whose narcissistic personality traits hinder treatment progress. In order to match specific patient’s needs, we integrated Cognitive Behavioral Therapy for cocaine dependence (Carroll, 1998) with tools of the Trans-Theoretical model of change (Prochaska & DiClemente, 1984) and with Mindfulness-Based Relapse Prevention interventions (Bowen, Chawla, & Marlott, 2013). Treatment initially involved: motivational support; detection of triggers and dysfunctional beliefs; functional analysis of consumption. Interventions were aimed at targeting the following goals: maintenance of abstinence; increase of emotional awareness; coping with craving and difficult emotions; behavioral and social reactivation; relapse prevention. Abstention from cocaine use were also maintained at the 6-months and 1-year follow-ups.
